If you're referred into SASH for 1-2-1 support (coaching, counselling, peer mentoring), you will be allocated a Care Coordinator, who will invite you to attend an assessment, either face to face or online, to discuss what support you need from SASH.
Our whole team at SASH are trained to support you with all of your sexual health needs. If topics covered in the assessment are sensitive or difficult for you, our Care Coordinators will approach this with compassion, without judgement and will guide you through it at a pace that suits you. Once completed, they can then create a tailored plan of support based on your needs.
Due to high demand or availability constraints, there might be a waitlist before beginning your 1-2-1 support. If this happens, your Care Coordinator will remain your main point of contact and will keep you updated throughout your journey.
At times, despite the variety of services available at SASH, we may find an external service could better meet your needs. In such instances, your Care Coordinator will help you access these services, ensuring a seamless transition for you.
With your consent, we will also collaborate with any other professionals involved in your care to advocate for your overall well-being.
Once you have finished your support plan, your Care Coordinator will talk to you about the next steps, which may include new referrals or assessing your readiness to conclude your support with SASH.
